The prescription form is an interactive form and can be copied as is and pasted into an e-mail to be sent to me.
Once you have pasted it into your e-mail, you can double click on each of the boxes and enter the numbers from your prescription.
IMPORTANT NOTE: Please use the plus or minus signs as indicated on your prescription. If there is no sign on your Sphere or Cylinder power, please assume that it is a "plus" and use the + sign when you enter the numbers. For example, instead of entering 1.25, please enter +1.25. Thank you!

You must enter the pupillary distance to assure the center of focus is directly in front of your eyes. Sometimes this information is missing and only recorded by whoever made your glasses.
If your e-m does not support HTML then alternate methods of sending your prescription will be needed. If you are not sure if your e-mail supports HTML, try to copy and paste it into your e-mail anyway to see if it will work. Scanning your prescription or taking a picture and sending it as a picture will work as will sending a copy by snail mail.
